In an intel document titled 'Weather' found during the level 'A Desperate Gamble', which is in London. The intel is the co-pilot's log entry and explains that Coxley lost control and broke formation flying right into enemy flak. The entire crew were killed and only the co-pilot survived. Despite Coxley's best efforts, it appears they were the victim of a bird strike.
